The Clay Evans $cholarship Foundation provides scholarships to students in the states of Illinois and
Tennessee. Students must meet the following criteria to be considered:

1. A graduating senior or have earned a GED by June 2023
2. A 2.0 GPA/“C” average or better
3. An active member of a church.
4. Submit the following :
- A completed online or paper application
- School Transcripts from the last four (4) years
- Three (3) letters of recommendations on official letterhead and signed from a

- School Official
- Church Official
- Community Service Official
- Copy of college acceptance letter
- Proof of Family Income

(W2 forms, Current Year’s Tax Return, SSI, Disability, Public Aid, Child Support, etc.)
