The Clay Evans $cholarship Foundation provides scholarships to students in the states of Illinois and
Tennessee. Students must meet the following criteria to be considered:
1. A graduating senior or have earned a GED by June 2025
2. A 2.0 GPA/“C” average or better
3. An active member of a church.
4. Submit each of the following for consideration (Only completed applications will be reviewed) :

A completed online application
School Transcripts from the last four (4) years
Three (3) letters of recommendations from each of the following:
- School Official
- Church Official
- Community Service Official
Copy of your college acceptance letter
Proof of Family Income
(W2 forms, Current Year’s Tax Return, SSI, Disability, Public Aid, Child Support, etc.)
