The Clay Evans $cholarship Foundation provides scholarships to students in the states of Illinois and Tennessee. Students must meet the following criteria to be considered:

1. A graduating senior or have earned a GED by June 2024

2. A 2.0 GPA/“C” average or better

3. An active member of a church.

4. Submit each of the following for consideration (Only completed applications will be reviewed) :

   • -  A completed online application

   • -  School Transcripts from the last four (4) years

   • -  Three (3) letters of recommendations on official letterhead and signed from a

     - School Official
     - Church Official
     - Community Service Official

   • -  Copy of college acceptance letter

   • -  Proof of Family Income

     (W2 forms, Current Year’s Tax Return, SSI, Disability, Public Aid, Child Support, etc.)

     The application and all supporting documents must be submitted via online submission by

     June 7, 2024

     Please note that the Scholarship Committee may conduct oral interviews.
